Job summary

Required is an active RN license, 2+ years in health care with care management exposure, and the ability to travel up to 40% locally. Strong communication and independent work skills are essential for success in diverse settings.

Qualifications

• RN license must be active in state of practice.
• 2+ years in health care with 1+ year in care management or LTSS.
• Experience with disabilities, chronic conditions, or substance use disorders preferred.

Responsibilities

• Complete comprehensive member assessments within timelines, including in-person visits.
• Facilitate waiver enrollment/disenrollment processes.
• Develop and monitor care plans with members and care network.
• Coordinate with multidisciplinary teams to ensure integrated care.
• Promote access to behavioral health and LTSS services across networks.
• Identify barriers to care and develop mitigation plans.
