Job summary

Molina Healthcare, Inc. is seeking an experienced Registered Nurse to join as a care manager, coordinating LTSS and behavioral health services. You will assess members, develop integrated care plans, and work closely with physicians, caregivers and the care team to achieve outcomes.

The role requires travel to member homes and sites, with strong emphasis on documentation, clinical guidance, and collaboration to ensure high-quality, cost-effective care delivery.

Qualifications

• Active RN license in state of practice.
• 2+ years in health care including care management and LTSS exposure.
• Valid driver’s license and reliable transportation.
• Ability to work independently with minimal supervision.
• Proficient with Microsoft Office.

Responsibilities

• Completes comprehensive member assessments within regulated timelines, including in-person home visits as required.
• Facilitates comprehensive waiver enrollment and disenrollment processes.
• Develops and implements care plans with members and care team to address needs and goals.
• Performs ongoing monitoring of care plans and suggests changes as needed.
• Promotes integration of LTSS with behavioral health and community resources.
• Assesses medical necessity and authorizes waiver services.
• Participates in ICT meetings for service decisions.
• Travel 25-40% locally based on state/contractual requirements.
