Job summary

Molina Healthcare seeks an RN care manager to support LTSS and care coordination for high-need members. You will collaborate with a multidisciplinary team to ensure quality and cost-effective member care, including home visits and waiver-related activities.

You will assess needs, develop care plans, monitor outcomes, and coordinate services across behavioral health and LTSS, while navigating regulations and funding sources. Strong communication and independent work are essential.

Qualifications

• At least 2 years in health care, incl. 1 year in care management/LTSS, and with disabilities.
• Active RN license in state of practice.
• Valid and unrestricted driver's license for job-related travel.
• Ability to operate proactively and be detail-oriented.
• Demonstrated knowledge of community resources.
• Ability to work independently with minimal supervision.
• Strong verbal and written communication skills.
• Proficiency with Microsoft Office suite.

Responsibilities

• Complete comprehensive member assessments within timelines, including in-person visits as required.
• Coordinate waiver enrollment/disenrollment processes.
• Develop and implement care plans with members and care networks.
• Monitor care plans and adjust interventions to achieve goals.
• Promote integrated services and continuity of care for LTSS and behavioral health.
• Identify barriers to care and provide support for psycho/social, financial, and medical issues.
• Lead or participate in ICT meetings for service decisions.
