Job summary

Molina Healthcare, Inc. is seeking a Nurse Care Manager to coordinate LTSS services for high-need members. You will perform comprehensive assessments, develop care plans with a collaborative network, and monitor progress toward outcomes.

This role requires strong communication, independence, and the ability to travel to member homes as needed. Responsibilities include waiver enrollment, care coordination across disciplines, and facilitating team meetings to authorize services.

Qualifications

• At least 2 years in health care including 1 year in care management or LTSS.
• Active RN license in state of practice.
• Bachelor's degree in a health care related field may be required in some states.
• Valid driver's license and reliable transportation for travel requirements.
• Proactive, detail-oriented with strong communication and self-motivation.

Responsibilities

• Complete comprehensive member assessments within regulated timelines, including home visits as required.
• Facilitate waiver enrollment and disenrollment processes.
• Develop and implement care plans with members and care network to address needs and goals.
• Monitor care plans, document interventions, and adjust as needed.
• Coordinate LTSS and behavioral health services to ensure continuity of care.
• Lead ICT meetings for approval/denial of services and collaborate across teams.
