Job summary

Position requires comprehensive member assessments, waiver management, and development of individualized care plans. Travel 25-40% may be necessary depending on state requirements and contracts.

Qualifications

• Active RN license in state of practice.
• At least 2 years healthcare experience incl. 1 year in care management.
• Experience with disabilities, chronic conditions, substance use disorders, and LTSS preferred.

Responsibilities

• Completes comprehensive member assessments within regulated timelines including in-person home visits when required.
• Facilitates waiver enrollment/disenrollment processes.
• Develops and updates care plans with members, caregivers, physicians and support networks.
