Job summary

ThedaCare is seeking a part-time RN Care Manager (0.5 FTE) at the Appleton location to coordinate multi-disciplinary care transitions. You will develop care plans, collaborate with the care team and family, and ensure timely access to services while balancing cost and quality for our patients.

Required ADN or BSN, Wisconsin RN license, and BLS. Eligible ADN candidates must have a related bachelor’s degree; CCM/ACM certification due within one year of hire.

Qualifications

• Associate Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN) required.
• ADN candidates must also hold a bachelor’s degree in a related field (e.g., Psychology, Social Work, Health Science, or equivalent).
• Candidates currently enrolled in a BSN completion program may also be considered.
• Current Wisconsin RN License; AHA BLS Provider Certification; CCM/ACM certification required within one year of hire for ADN candidates.

Responsibilities

• Coordinate care across disciplines to transition patients to appropriate care levels.
• Develop and implement care plans to achieve high-quality outcomes and cost efficiency.
• Communicate with patients, families, and care teams to address needs and barriers.
• Facilitate access to primary care, specialists, and community resources.
