Job summary

Community Care in Milwaukee, WI is hiring a Case Manager/Care Manager for a day shift in our Family Care Program. The role involves managing caseloads, conducting comprehensive assessments, and coordinating care plans to support elderly individuals and adults with disabilities in the community.

This full-time position offers a Monday–Friday schedule from 8:00 a.m. to 4:30 p.m., with an $8,000 sign-on bonus and a strong emphasis on teamwork, education, and compassionate, person-centered care.

Qualifications

• Education: Bachelor’s Degree in Human Services or closely related field required.
• Licensure: Functional screener certification preferred; eligibility required.
• Experience: Minimum 3 years working with older adults or persons with disabilities; 3 years in community nursing preferred.

Responsibilities

• Manage caseload of Family Care Program members.
• Complete comprehensive clinical assessments.
• Develop member-centered care plans and provide ongoing care coordination and education.
• Travel locally using personal vehicle with valid license and insurance.

Community Care is hiring a Case Manager/Care Manager Day Shift for our Family Care Program in Milwaukee, WI. Offering an $8,000 sign-on bonus! This is a Monday through Friday position, 8:00 a.m. to 4:30 p.m.

Case Manager/Care Manager Job Responsibilities:

• Manage caseload of Family Care Program members
• Complete comprehensive clinical assessments
• Working collaboratively to develop, member centered care plans, provide on-going care coordination, member education, quality assurance and monitoring of services.
• Travel,Regular local travel, requiring use of personal vehicle, valid driver's license, and proof of required insurance coverage.

Case Manager/Care Manager Job Requirements:

• Education: Bachelor's Degree in Human Services or closely related field required.
