Qualifications

• Current Registered Nurse license in the state of practice.
• Minimum 2 years of clinical nursing experience.

Responsibilities

• Conduct comprehensive assessments and create care plans.
• Collaborate with patients, families and a multidisciplinary team to meet goals.
• Coordinate discharge planning and ensure regulatory compliance.
• Perform continuum of care and transfer coordination across units.
• Deliver case management to address healthcare access, finances, housing, and more.
• Document UM activities and CMS notices on time.
