Advocate Health is seeking an RN Care Manager for inpatient services at St. Luke’s in Milwaukee, WI. You will assess patients, coordinate discharge planning, and work with a multidisciplinary team to ensure safe transitions and regulatory compliance.

Requirements include a Registered Nurse license, a BSN, and a minimum of two years' clinical nursing experience. This is an on-site, full-time role with a weekend/holiday schedule and competitive hourly pay.

• Coordinate transfers and handoffs across units and settings.
• Deliver case management services using community resources.
• Initiate referrals and document discharge planning and UM activities.
