Job summary

GBMC Hospital is seeking a part-time RN Care Manager to oversee inpatient care coordination and discharge planning at our Towson, MD campus. The role collaborates with interdisciplinary teams and payers to optimize patient outcomes and reimbursement.

Ideal candidates have a BSN or enrolled BSN in the next three years, plus 2 years of case management experience in acute care or similar settings. Strong communication and problem-solving skills are essential.

Qualifications

• BSN or ASN with BSN in progress and expected graduation within three years.
• Two years of case management including discharge planning and readmission initiatives.
• Experience in acute care preferred.

Responsibilities

• Assess patients within a caseload to identify status, needs, financial resources, and goals.
• Coordinate discharge planning and post-discharge referrals.
• Review admission criteria and develop stay or discharge plans.
• Coordinate with payers and care teams to ensure utilization compliance.
• Lead care management meetings and educate staff.
• Collaborate with social work on discharge planning.
