Job summary

GBMC is seeking an experienced care management professional to oversee inpatient process coordination, discharge planning, and cross-disciplinary collaboration. You will work with physicians and payers to optimize reimbursement and patient outcomes across the continuum of care.

Qualifications include a BSN or ASN with BSN in progress, two years of case management experience, and Maryland RN licensure. A strong focus on communication, critical thinking, and problem solving is essential.

Qualifications

• Two years of Case Management experience including discharge planning and working with readmission initiatives.
• Diverse, progressive experience in acute care and/or other settings within the continuum preferred.

Responsibilities

• Assess patients within a defined caseload to identify clinical status, needs, resources, and care goals, considering insurance and reimbursement.
• Conduct frequent rounds, coordinate with physicians and health care team to implement the patient-centered plan.
• Review admission and stay criteria and develop concurrent stay or discharge plans per guidelines.
• Plan and coordinate discharge needs, referrals, and post-discharge arrangements with community resources.
• Maintain knowledge of payer requirements and coordinate services with managed care and other payers; obtain authorizations as needed.
• Lead and participate in health care team meetings to optimize discharge planning for complex cases.
