Job summary

GBMC HealthCare seeks an experienced RN Case Manager to coordinate inpatient care, discharge planning, and utilization review to optimize quality and cost‑effective outcomes. You will collaborate with physicians, payers, and the interdisciplinary team to ensure appropriate admission, stay criteria, and post‑discharge placement.

Requires BSN or ASN with BSN enrollment and two years of case management experience; strong communication and critical thinking skills are essential.

Qualifications

• BSN or ASN with BSN enrollment within three years of hire.
• Two years of Case Management experience including discharge planning and readmission initiatives.
• Knowledge of care management theories, principles, and standards.

Responsibilities

• Assess patients to identify clinical status, needs, and care goals; coordinate with payers.
• Coordinate discharge planning and post-discharge placement with interdisciplinary team.
• Serve as liaison among patient, physician, and care management team to ensure continuum of care.
• Ensure adherence to admission and stay criteria and facilitate utilization reviews.
