A Master’s in Social Work and Maryland licensure (LGSW/LCSW/LCSW-C) are required, along with at least three years of

• Master's in Social Work required.
• MD LGSW, LCSW or LCSW-C licensure required.
• 3 years clinical social work experience, preferably in a hospital setting.
• Discharge planning experience preferred.
• Knowledge of social services, Medicaid and related regulations.
• Ability to coordinate an interdisciplinary team and assess complex patient needs.

Responsabilités

• Provide psychosocial assessments and complex discharge planning.
• Coordinate services, referrals and resource access for patients.
• Facilitate support groups and provide consultation within the care team.
