Job summary

Qualifications

• Nursing degree from an accredited BSN program.
• RN license in MA or eligible and start date CCM or related certification preferred within 24 months of hire.
• 1–3 years acute inpatient care management experience required.
• 2 years in utilization management or case management preferred.

Responsibilities

• Assess patient needs and plan care with multidisciplinary team.
• Coordinate/discharge planning and resource utilization.
• Communicate with physicians, nurses, and payers to ensure timely, appropriate care.
• Maintain documentation in patient records and quality screens.
