Boston Medical Center is seeking a Registered Nurse Care Manager for the 4E department. The role involves a collaborative process to assess and coordinate services, ensuring timely, cost-effective outcomes for patients.

The position requires RN licensure in Massachusetts and 3–5 years of clinical experience, including 1–3 years in acute inpatient care management. BSN preferred; BSN may be pursued within 4 years.

Qualificações

• Graduate of an accredited BS program in Nursing preferred.
• Nursing degree: Diploma, ASN or BSN (preferred), Ability to obtain BSN within 4 years
• Licensed to practice professional nursing as a Registered Nurse in the Commonwealth of Massachusetts.

Responsabilidades

• Assess, plan, implement, monitor, and evaluate options and services for patients’ health needs.
• Identify appropriate providers and facilities; coordinate care across continuum.
• Manage resources to optimize value for patients and reimbursement sources.
