Qualifications

• Current License to practice as a Registered Nurse in the State of Wisconsin.
• Two or more years of skilled nursing experience preferred, ideally in gerontology/disabilities and/or home care.
• Ability to lift up to 25lbs.

Responsibilities

• Conduct comprehensive assessments of members’ outcomes, needs, and risks.
• Coordinate acute and primary care services for members.
• Develop and monitor member-centered plans of care.
