Job summary

Lakeland Care, Inc. is seeking a Registered Nurse (RN) Care Manager to join an interdisciplinary team dedicated to serving frail elders and adults with disabilities in Wisconsin. The RN Care Manager will oversee care management and service coordination, ensuring quality, person-centered care in line with member needs.

Qualified candidates must have a current RN license in Wisconsin and preferably a Bachelor's degree in nursing. Experience with skilled nursing, particularly in gerontology or home care, is highly desirable. The position involves in-home visits and requires a valid driver's license.

Qualifications

• Current License to practice as a Registered Nurse in the State of Wisconsin.
• Two or more years of skilled nursing experience preferred, ideally in gerontology/disabilities and/or home care.
• Current driver's license with acceptable driving record.
