Job summary

Qualifications

Responsibilities

• Facilitates communications among patient/family, care teams, and resources to resolve barriers to timely care.
• Collaborates with care teams to ensure quality, efficient, and cost-effective services.
• Uses evidence-based approaches to engage patients and set SMART goals.
• Identifies barriers to learning or delivery of care and develops plans.
• Manages CM caseload per department expectations with timely documentation.
• Partners with at‑risk patients across diagnosis, treatment and follow-up to ensure continuity of care.
• Coordinates referrals and patient information between facilities, providers, and community resources.
