Advocate Health in Georgia is seeking a part-time Care Manager to assess, plan, coordinate, and evaluate patient services across acute care, home health, and discharge planning. Schedule varies with weekends and holidays, and compensation aligns with experience and needs.

The role requires an RN license, BSN preferred, and minimum three years in care management or related fields, with familiarity in IT systems like EHR and learning platforms.

Qualifications

• Completion of an accredited nursing educational program, BSN preferred.
• Current registered nursing license. Eligibility to manage patients in a facility outside of the state of applicable state requires a compact License.
• Minimum of three years of recent experience in acute care, home health, case management or care management. Experience as a Care Manager preferred.
• Experience with IT solutions such as electronic health record, learning management or disease/care management systems a plus.

Responsibilities

• The Care Manager assesses needs, plans, coordinates, and evaluates services of patients with the goal of equipping and empowering individuals and their families to access resources and adopt healthy lifestyles.
• Works collaboratively with the multidisciplinary care team including providers, pharmacists, social workers, behavioral health specialists, and nurses to achieve timely management.
