Job summary

Medical University of South Carolina is seeking a Nurse Case Manager II to coordinate care plans and ensure safe, timely discharges. Working under the Case Management & Care Transitions team, you will drive discharge planning and resource utilization across the care continuum.

Hours are 40 per week, days Monday-Friday with occasional weekends/holidays, salaried position. Requires RN licensure in SC or compact state and BSN with leadership and CM experience preferred.

Qualifications

• Bachelor of Science in Nursing required.
• Minimum of 3 years as Registered Nurse.
• Case Management experience preferred or certification.

Responsibilities

• Coordinate a plan of care to achieve clinical outcomes.
• Promote safe and timely discharge across the care continuum.
• Serve as a clinical expert in discharge planning and resource utilization.
