Job summary

Mercy is seeking a Care Manager who, as part of an interdisciplinary team, will assess, plan, advocate for, and coordinate care from admission to discharge to ensure safe transitions after hospitalization.

The role emphasizes care management with emphasis on transition planning, length of stay oversight, performance metrics, and adoption of best practices within the Mercy mission and values. Qualified candidates include RNs with current Missouri licensure and BLS certification.

Qualifications

• Graduate of an accredited School of Nursing.
• Current License in the state of employment.
• BLS (CPR) at hire date, or within 90 days of hire.
• Care management or utilization management experience preferred.

Responsibilities

• Assess, plan, advocate for, and coordinate care from admission to discharge.
• Ensure safe transition post hospitalization with length of stay oversight.

Job Profile Summary

Position Details

• Graduate of an accredited School of Nursing, required

Experience

• 2-3 years acute care hospital setting, preferred.
• Care Management or Utilization Management experience, preferred

Licensure

• Current License in the state of employment, required

Certifications

• BLS (CPR) at hire date, required, or within 90 days of hire
• Certification in Case Management, Preferred
