Mercy is seeking a Care Manager to lead discharge planning from admission through discharge, collaborating with the health care team to ensure safe patient transfers.
The role supports patients with complex psychosocial needs and promotes care management best practices within Mercy’s standard of care.

The Care Management model provides effective transition planning and length of stay oversight while encompassing quality patient care, patient safety, and financial components; promotes integration of a seamless care model; coordinates patient throughput; encourages healthcare team collaboration and coordination as evidenced by metrics; and optimizes performance and adoption of best practice. The Care Manager, as part of the Care Management team, facilitates the discharge planning process from admission to discharge in collaboration with the healthcare team. In addition, the Care Manager may intervene with patients who have complex or high-risk psychosocial needs as assigned. Performs duties and responsibilities in a manner consistent with the Mercy mission, values, and Service Standards.
Day Shift: Saturday thru Tuesday - 32 hours per week; (8:00am-4:30pm)
Education Graduate of an accredited School of Nursing, required Current License in the state of employment, required Bachelor's Degree of Science in Nursing, minimum required Experience 2-3 years acute care hospital setting, preferred Care Management or Utilization Management experience, preferred Certification/Registration BLS (CPR) at hire date, Minimum Required, or within 90 days of hire Certification in Case Management, Preferred.

Our Mercy health system was founded by the Sisters of Mercy in 1986. But our heritage goes back more than 195 years. It began with an Irish woman named Catherine McAuley, who was determined to help women and children in Dublin experiencing poverty. Though Catherine had a modest upbringing, she received an unexpected inheritance that allowed her to fulfill that dream. In 1827, she opened the first House of Mercy in Dublin, intending to teach skills to women and educate children. Many volunteers came to help. A few years later, Catherine founded the Sisters of Mercy, one of the first religious orders whose Sisters were free to walk among the poor and visit them in their homes. In 1843, the Sisters of Mercy came to the United States. In 1856, they traveled to St. Louis. In the years to come, they would establish hospitals throughout the Midwest. Many of these would join together in 1986 as Mercy.
