Intermountain Health is seeking an RN Hospital Care Manager I to deliver comprehensive care management across inpatient and outpatient settings in Colorado. You will assess, plan, and coordinate patient-centered care with multidisciplinary teams to optimize outcomes.

This role emphasizes motivational interviewing, patient education, and advocacy, with focus on transition care and regulatory compliance. BSN is required within four years if not already held.

Formación

• Current Registered Nurse (RN) license in state of practice.
• BSN from an accredited institution; BSN must be obtained within 4 years if not already held.
• Inpatient hospital nursing experience; familiarity with hospital terminology and processes.
• Understanding of disease management, barriers to care, and variations in treatment.

Responsabilidades

• Identify patients using screening criteria and conduct comprehensive evaluations.
• Develop, adjust, and coordinate care plans addressing medical and social needs.
• Guide self-management using motivational interviewing and coaching.
• Provide education to healthcare teams and patients; advocate for patient rights and self-care.
• Collaborate with multidisciplinary teams to ensure timely, high-quality care.
• Lead initiatives to improve care transitions and policy adherence.
• Provide Goals of Care education for informed decision-making.
• Track key metrics, analyze trends, and ensure regulatory compliance.
