Job summary

HiredFirst is seeking a dedicated Home Health Registered Nurse (RN) to join a patient-centered care team delivering high-quality nursing in patients' homes across CA, HI, OR, and WA. You will assess, plan, and implement individualized care while educating patients and families.

The role requires an active RN license in the state, CPR/BLS certification, a valid driver's license, reliable transportation, and the ability to work independently with a caseload.

Qualifications

• Home health or acute care nursing experience preferred

Responsibilities

• Conduct comprehensive patient assessments and develop individualized care plans
• Provide skilled nursing care and ongoing evaluations in patients' homes
• Educate patients and caregivers on treatment plans, medications, and preventive care
• Collaborate with interdisciplinary healthcare teams to ensure coordinated patient care
• Maintain accurate and timely clinical documentation in compliance with agency standards
• Monitor patient progress and adjust care plans as needed
• Participate in on-call rotations as assigned
