Job summary

Physicians Choice is seeking a Registered Nurse Home Health Case Manager for a full-time, in-home care role in Oregon. The position involves direct patient visits, developing and updating personalized care plans with physicians, and delivering high-quality home health services.

The role requires an Oregon RN license, valid driver's license, CPR certification, and proven home health experience. Travel will be frequent, with emphasis on compassionate, patient-centered care and adherence to safety

  • Physical endurance, capable of frequent travel and navigating various home environments.
  • Knowledge of best practices in home healthcare and patient-centered care.
  • Empathy and a deep commitment to patient welfare and sensitivity to their needs.

Responsibilities

  • Evaluate and assess the patient's health condition during each home visit.
  • Create and update comprehensive, individualized care plans in collaboration with physicians and other healthcare professionals.
  • Ensure adherence to all safety protocols and maintain high standards of hygiene and cleanliness during home visits.
  • Administer medication as prescribed and monitor responses to treatment.
  • Educate patients and their families on disease prevention and health promotion.
  • Provide emotional and psychological support to patients and their families.
  • Maintain detailed records of patient's health progress and treatment plans.
  • Coordinate with healthcare providers to ensure patient care is aligned with overall health goals.
  • Manage a caseload of patients, prioritizing needs based on severity and urgency.
  • Respond to emergency health issues as they arise and take decisive steps to address them.
  • Advocate for patients' needs and assist in navigating the complexities of home healthcare.
  • Conduct regular follow-up visits to assess progress and adjust care plans as needed.
