Job summary

Talented Medical Solutuins in Clearlake, CA seeks an experienced RN Care Manager for Home Health. This temporary assignment runs April 27, 2026 to July 5, 2026, 40 hours per week, day shift. You will coordinate care in patients' homes and serve as a key link among patients, families, and the care team.

The role requires an active CA RN license, Home Health experience, HCHB software and OASIS documentation experience, and BLS. A BSN and case management credentials are a plus.

About the Role

As an RN Care Manager in Home Health, you’ll serve as the vital link between patients, families, and the healthcare team—ensuring coordinated, high-quality care in the comfort of patients’ homes.

Important: This is HOME HEALTH (not hospital) — prior experience is required.

What You Bring

Required:

Active Registered Nurse (RN) license (California or eligible)

Home Health experience (MUST HAVE)

HCHB (Homecare Homebase) software experience (REQUIRED)

OASIS documentation experience (REQUIRED)

BLS certification

Preferred:

Bachelor’s Degree in Nursing (BSN)

Case management experience or certification
