Job summary

AIDS Healthcare Foundation in Beverly Hills is seeking an RN Medical Care Manager to apply strong nursing and Ryan White Program knowledge within our Healthcare Center. You will collaborate with clinicians, the Care Management team, and other providers to ensure each Ryan White patient receives education, follow-up, and access to resources.

Responsibilities include Needs Assessments, Care Plan development, and ongoing education so patients understand their diagnoses and medications.

Qualifications

• BSN or LPN with 1 year of case management experience.
• Knowledge of Ryan White Services preferred.
• Education and follow-up with clients is required.

Responsibilities

• Conduct Needs Assessments for Ryan White patients.
• Develop and implement Care Plans.
• Provide ongoing education about diagnoses and medications.
• Coordinate with clinicians and Care Management team for holistic care.
• Perform retention calls to minimize gaps in services.
