AIDS Healthcare Foundation in Beverly Hills is seeking an RN Medical Care Manager to apply strong nursing and Ryan White Program knowledge within our Healthcare Center. You will collaborate with clinicians, the Care Management team, and other providers to ensure each Ryan White patient receives education, follow-up, and access to resources.
Responsibilities include Needs Assessments, Care Plan development, and ongoing education so patients understand their diagnoses and medications.
A RN Medical Care Manager contributes to AHF by utilizing strong
nursing clinical skills and understanding of the Ryan White Program
within their specific area. With a caring and knowledgeable focus,
you will work with our physician and other providers, the Care
Management team, and the Healthcare Center team so that every
Ryan White patient receives the support they require and access to
available resources. This will include providing education and followup
with the clients. Working directly within the Healthcare Center,
you will meet with patients and complete Needs Assessments,
develop Care Plans, and provide ongoing education to clients so they
understand their diagnoses and their medications. Working with your
team allows for full collaboration to meet all aspects of your patient’s
health and psychosocial needs. In this way, you will support the
patient’s ability to understand their health status better to work to
achieve their healthcare goals. The RNMCM will consult with
clinicians to develop treatment regimens that ensure optimal clinical
outcomes, address treatment adherence education needs and provide
ongoing education for clients to self-manage their HIV by keeping
their healthcare appointments and taking their prescribed
medications. You will also conduct ongoing retention calls to ensure
no gap in retention efforts and to uphold radical cooperation across
divisions.
Licenses and Certifications: Bachelor’s degree in Nursing (BSN), or
License Practical Nurse (LPN) with a minimum of 1 year of Case
Management experience.
Knowledge of Ryan White Services is preferable.
