Job summary

Greater Lawrence Family Health Center in Lawrence, MA is seeking an RN Care Manager to improve clinical and operational performance for patients with HIV, viral Hepatitis and Substance Use Disorder. The nurse will organize, plan and coordinate care with the healthcare team, providing care services such as follow-up visits, education on HIV and hepatitis, and adherence support.

The role emphasizes access to care and retention in care, aligning with standards of care.

Qualifications

• Massachusetts RN license required.
• Current BLS certification.
• Minimum 2 years' experience in case management or care coordination.
• Bilingual Spanish/English preferred.
• Strong communication, organization, and flexibility.
• Valid Massachusetts driver's license and access to reliable transportation.

Responsibilities

• Conduct intake on new patients and develop a culturally competent care plan.
• Provide HIV and viral hepatitis education; counseling on harm reduction and adherence.
• Lead HIV and Viral Hepatitis clinics; manage scheduling, flow, and coordination of services.
• Document each patient contact in the EMR per protocol.
• Manage needs of complex patients using physician-approved protocols and order sets.
• Provide direct patient care when indicated, including immunizations and phlebotomy.
