Qualifications

Responsibilities

• Identify patients who may benefit from high utilizer care management.
• Coordinate across primary care, behavioral health, specialty care, and hospitals.
• Provide transitional care management after ED visits or hospitalizations.
• Assist patients with barriers to access (transportation, housing, insurance, food).
• Document care plans and outcomes in the EHR.
• Participate in team meetings and quality improvement initiatives.
