Job summary

Molina Healthcare is seeking a Care Manager RN to support LTSS delivery in Michigan, primarily Wayne and Macomb counties. The role involves member-facing duties with anticipated travel around 50 percent and collaboration with multidisciplinary teams to coordinate care across the continuum.

Responsibilities include conducting comprehensive assessments, developing individualized care plans, and monitoring progress while navigating waiver services and funding.

Qualifications

• At least 2 years in health care with care management or managed care experience.
• Experience working with persons with disabilities, chronic conditions, or LTSS is preferred.
• RN license must be active and unrestricted in practice state.

Responsibilities

• Complete comprehensive member assessments within regulatory timelines, including in-person home visits.
• Facilitate waiver enrollment/disenrollment processes.
• Develop and implement care plans with members and care team.
• Monitor care plan effectiveness and adjust as needed.
• Promote integration of services to improve continuity of care.
• Assess medical necessity and authorize waiver services.
