Job summary

Careforth, Inc. is seeking an RN Care Manager or Care Manager to provide personalized care to caregivers and loved ones. No night shifts, weekends, or on-call requirements.

You will assess needs, develop care plans, conduct home visits, and ensure high-quality care for families across the Boston area. You will travel throughout Dorchester, Roxbury, Hyde Park, and surrounding areas, building strong caregiver relationships and influencing positive care outcomes.

Qualifications

• Active RN license in the state of employment or a bachelor’s degree for Care Manager candidates.
• Minimum two years of direct care experience with elders or adults with disabilities.
• Ability to travel within assigned geographic area; valid driver’s license and automobile insurance required.
• Experience in case management, care planning and assessment for eldercare/disabled populations in healthcare settings.
• Proficiency in common software and web-based applications (e.g. MS Office).

Responsibilities

• Conduct home visits in assigned territory to assess safety, health changes, and provide clinical coaching.
• Provide ongoing coaching to caregivers via app, with resource referrals and best practice reinforcement.
• Develop trusting relationships with caregivers to reduce burnout.
• Document and review records for accuracy and timeliness per regulatory standards.
• Perform other duties as assigned.
