Advocate Health in Concord, NC seeks an experienced Care Manager to coordinate care across patient/family, clinicians, and community resources, ensuring timely access and quality outcomes. You will document activities in the EMR and support care transitions with a patient-centered approach.

Ideal candidates hold a BSN, an RN license, and 5 years of clinical nursing or 1–2 years in care management, with strong communication and organization skills. Some travel between sites may be required.

Formación

• Bachelor's Degree in Nursing or related field required.
• Registered Nurse license to practice in the state.
• 5 years of clinical nursing or 1–2 years in care management typically required.
• Experience with Epic EMR is preferred.
• Strong communication and collaboration with interdisciplinary teams.

Responsabilidades

• Facilitates communications among patient/family and care teams to resolve barriers to timely care.
• Collaborates with primary, hospital, post-acute teams and partners for quality, efficient care.
• Uses evidence-based approaches to activate patients and set SMART goals.
• Identifies barriers to learning and care delivery and reports findings with a plan.
• Manages CM caseload independently per department expectations.
• Partners with at-risk patients through diagnosis, treatment and follow-up for continuity.
• Coordinates patient information and transitions among facilities, providers and resources.
• Coordinates internal/external referrals as needed.
• Ensures age-appropriate care per department policies.
