Qualifications

Responsibilities

• Identifies and coordinates patient care needs to support safe transitions.
• Documents plans of care and handovers for level-of-care changes.
• Promotes resources and services to meet patient health goals.
• Supports and participates in discharge planning and rounds.
• Maintains knowledge of resources and payer practices.
