Job summary

Mercy Health is seeking an RN Care Manager at St. Anne Hospital to coordinate patient care, support safe transitions, and collaborate with a multidisciplinary team. You will identify needs, assess plans, and promote timely and appropriate resources across the continuum of care.

The role requires an RN license, BLS, and at least 1 year of clinical experience, with 3 years in acute care preferred. A CCM/ACM certification is a plus for advancement.

Qualifications

• Bachelor of Science in Nursing is preferred; RN license required.
• BLS Basic Life Support is required.
• Certified Case Manager (CCM) or ACM certification preferred.
• 1 year of clinical experience required; 3 years acute care preferred.
• Ambulatory or post-acute care coordination experience is a plus.

Responsibilities

• Identify and prioritize patients needing care management.
• Maintain clear, concise documentation in patient records.
• Coordinate transitions of care across the continuum.
• Support and promote timely testing and treatment.
