Job summary

Atrium Health Floyd Medical Center is seeking a Case Manager to apply the nursing process to coordinate care across disciplines. You will advocate for patients, collaborate with physicians, nurses, therapists, and ancillary staff to ensure timely, cost-effective outcomes.

Requirements include RN licensure in GA or AL, BSN preferred and MSN encouraged, with at least five years of experience in case management. Full-time, 40 hours weekly, benefits eligible, on-site in Georgia/Alabama region.

Qualifications

• Graduate of an accredited School of Registered Professional Nurses, BSN preferred, MSN encouraged.
• Minimum of five years of experience in the care of assigned patient population.
• Current Registered Nurse license in GA or AL; Case Management certification encouraged.

Responsibilities

• Utilizes the nursing process to develop, implement, and evaluate case management outcomes.
• Collaborates with a patient care team to coordinate services and advocate for patients.
• Educates, consults, and assists in managing identified populations to improve outcomes and reduce costs.
