We are seeking a dedicated RN to promote and facilitate effective management of hospital resources from admission to discharge, coordinating with clinical teams to identify patients who will benefit from care coordination and utilization management services. The role emphasizes handling complex cases, maintaining regulatory compliance, and educating patients and families about plans of care while working

Qualificações

• Graduate of an approved school of nursing.
• Florida RN license required.
• Handle with Care certification within 90 days of hire.

Responsabilidades

• Assess patients for care coordination needs.
• Coordinate care from admission to discharge with the clinical team.
• Ensure compliance with regulatory standards and guidelines.
• Lead and participate in multidisciplinary patient care conferences.
