Job summary

Orlando Health Bayfront Hospital in St. Petersburg, FL is seeking an RN Care Manager, Acute to coordinate patient care across the continuum, ensuring safe, efficient utilization of resources and quality outcomes.

The role partners with physicians, nurses, social workers and therapists to assess discharge needs, develop Care Transition plans, monitor length of stay, and facilitate transitions to post-acute care. Weekend and holiday rotations apply in a day shift, full-time schedule.

Qualifications

• Graduate of an approved school of nursing.
• Must hold and maintain a current Florida RN license.
• Three (3) years of experience in chronic disease management, care management, care coordination, utilization management, or acute clinical care.

Responsibilities

• Coordinate patient care across the continuum to ensure quality outcomes.
• Assess discharge planning needs and develop care transition plans.
• Collaborate with physicians, nurses, social workers, therapists, and ancillary services.
• Monitor length of stay and address barriers to timely discharge.
• Ensure regulatory standards and documentation meet Joint Commission requirements.
• Lead and participate in multidisciplinary patient care conferences.
