Job summary

Corewell Health in ZeelandHospital is seeking an experienced RN for full‑time Case Management. You will plan, coordinate, and manage care for diverse patient populations, liaising with PCPs, specialists, and payers to ensure high‑quality, cost‑effective outcomes.

The role requires a BSN or higher degree with Michigan RN licensure, 2–5 years in clinical nursing/case management, and BLS. Day shift, 40 hours/week, on‑site at Zeeland Hospital, with comprehensive benefits.

Qualifications

• Bachelor's Degree or equivalent in nursing or a health-care related field.
• BSN must be obtained within 7 years for those who start in this RN role on or after January 1, 2017.
• 2 years of relevant clinical nursing and/or case management, managed care, or related field experience.
• 5 years of relevant clinical nursing and/or case management, managed care, or related field experience.
• Registered Nurse (RN) – State of Michigan.
• Case Manager, Certified (CCM).
• Basic Life Support (BLS).

Responsibilities

• Plans and prioritizes care for individuals and populations within a managed care environment.
• Demonstrates nursing expertise and manages a caseload of patients with varying complexity.
• Assesses internal and external referrals to identify patient needs and resources.
• Develops a plan of care and makes recommendations to PCPs and other health‑care team members.
• Assesses educational needs and implements appropriate teaching strategies.
• Coordinates services with patients, providers, and payers to improve access and cost‑effective care.
• Measures outcomes, interprets data trends, and documents patient data per guidelines.
