The incumbent collaborates with PCPs, specialists, and payers to improve access and outcomes while maintaining cost-effectiveness. Requirements include a Michigan RN license and a BSN; CCM certification is preferred.

Qualifications

• Bachelor's Degree or equivalent Nursing or healthcare related field required.
• BSDN within 7 years for those starting in this RN role on or after January 1, 2017 for Corewell Health Hospitals.
• RN license in Michigan required on hire.
• CCM certification preferred on hire.

Responsibilities

• Plans and prioritizes care for individuals and population health in managed care.
• Assesses referrals to identify patient needs and resources.
• Develops care plans and coordinates with PCPs and specialists.
• Educates patients and families and coordinates services across the continuum.
• Measures outcomes and analyzes data trends for groups of patients.
