Job summary

Corewell Health in Detroit is seeking an RN Manager of Clinical Homecare to oversee personnel and patient care services, ensuring compliant and safe delivery of care. The role provides backup to the Administrator/ DON and requires oversight of field staff and scheduling.

The candidate will coordinate Start of Care, monitor outcomes, and lead quality improvement initiatives while collaborating with billing on authorization processes. This is a hands-on leadership role within our home health team.

Qualifications

• Minimum of three (3) years of RN or PT experience in an acute care setting.
• Strong critical thinking skills with the ability to problem solve clinical needs.
• Exemplary communication skills with customers and colleagues.
• Ability to remain calm, be patient and compassionate while working with patients.

Responsibilities

• Ensure quality and safe delivery of home health services.
• Collaborates with intake, provides daily direction, and manages referral volume based on staffing.
• Oversees liaison workflow to ensure timely completion and identifies education needs for field staff.
• Ensure Start of Care compliance.
• Focus on outcomes and monitor as assigned.
• Implement quality improvement programs in the field.
• Participate in on-call rotation and manage after-hours staffing and patient issues.
• Manage PTO, call offs, and productivity reviews to meet staffing needs.
• Maintain active RN or PT license in the state of employment.
• Provide data for performance evaluations and support staff development.
• Maintain positive relationships with referral sources including physicians and hospitals.
