Qualificações

• Associate degree in nursing is required.

• 2–4 years of care management experience is preferred.

Responsabilidades

• Coordinate discharge planning and transitions of care.
• Review medical records for appropriateness and medical necessity.
• Collaborate with interdisciplinary teams (IDT) for effective communication.
• Develop and implement discharge plans and coordinate post‑hospital placement.
• Refer cases to physicians or managers when criteria aren’t met.
• Liaise with community agencies to facilitate smooth transitions.
• Maintain documentation of case management activities and compliance.
• Assist patients, families, and physicians with discharge planning and post‑hospital care options.
