Job summary

AdventHealth in Orlando, FL is seeking a registered nurse to join our discharge planning team. In this full-time role, you will assess patients, coordinate with the care team, and develop discharge plans to ensure timely and safe transitions from hospital to post-acute care.

Ideal candidates hold an Associate’s degree in Nursing, have 1+ year of nursing experience, and possess an active RN license. This position offers day shifts, benefits from day one, and opportunities for professional growth.

Qualifications

• RN license required
• Associate’s degree in Nursing required
• Bachelor’s degree preferred
• Minimum 1 year of nursing experience

Responsibilities

• Assess patients and families for discharge planning needs
• Review medical records and incorporate clinical, social, and financial factors into the transition of care plan
• Develop discharge plans with contingency plans throughout the hospital stay to ensure timely care coordination
• Coordinate post‑acute care services and facilities for social needs
• Leverage technology and communicate with post‑acute care services to ensure continuity of care and complete medical records
• Participate in multidisciplinary rounds to identify resources for discharge
• Educate patients and families about emotional, social, and financial impacts of illness and advocate for patient rights
• Assess readmitted patients for the patient’s and family’s perceived reasons for readmission
• Organize and facilitate patient and family care conferences with the multidisciplinary team
• Document discharge planning evaluation, ongoing assessment, discharge plans, barriers to progression, and patient needs
• Communicate with Payors for authorization for post‑acute care as needed
• Other duties as assigned
