Job summary

BMCHS is seeking an RN Care Manager to assess, plan, implement, monitor, and evaluate care for a designated patient caseload. You will coordinate multidisciplinary care, promote efficient discharge planning, and ensure quality, cost-effective outcomes.

You will collaborate with physicians and payers, document plans, and guide the patient through the continuum of care, using established care pathways and utilization criteria.

Qualifications

• BSN required; CCM preferred.
• Experience in acute care management or utilization management.
• Proficient in medical terminology and care pathways.

Responsibilities

• Assess patient needs and develop a comprehensive care plan.
• Coordinate care across disciplines and settings.
• Monitor LOS and resource use; ensure timely discharge planning.
• Communicate with physicians, payers, and care team.
• Document care plans and outcomes in medical records.
• Facilitate referrals to home health, hospice, or equipment.
