Greater Baltimore Medical Center, Inc. is seeking a Nurse Case Manager to coordinate inpatient care, discharge planning, and interdisciplinary collaboration to promote quality, cost-effective outcomes.

You will work with physicians, payers, and the care team to maximize reimbursement, ensure appropriate utilization, and help patients transition safely through the continuum.

Current Maryland RN license or enrollment in BSN program required; two years of case management experience preferred.

Competenze

• Two years of case management experience including discharge planning.
• Diversified, progressive experience in acute care or continuum preferred.
• Maryland RN licensure required or eligible for licensure.

Mansioni

• Assess patients within a defined caseload to identify clinical status, needs, and care goals.
• Coordinate discharge planning and post-discharge referrals and resources.
• Collaborate with physicians, payers, and care team to implement patient-centered plans.
• Review admission/continued stay criteria and ensure payer/clinical guideline alignment.
• Educate physician, nursing, and ancillary staff on care management and documentation requirements.
