Kaiser Permanente Northwest is seeking an Inpatient Care Manager RN to assure the appropriate level of care, coordinate transitions, and support discharge planning for complex patients. You will work with physicians and the care team to optimize resources while maintaining quality.

Responsibilities include thorough assessments, developing care plans, and ensuring safe transfers across settings. Requires Oregon RN license and BLS within 3 months; part-time, onsite in Salem, OR.

Qualifications

• Minimum of two years RN experience in Med/Surg, ICU, ED, Home Health, SNF, Hospice, LTAC or Inpatient Rehab.
• Experience with Utilization Management and discharge planning processes.
• Ability to document assessments clearly and completely for care transitions.

Responsabilités

• Coordinates post-discharge patient care to ensure timely, effective hospital discharge.
• Documents assessments that are thorough, timely, age-appropriate, and reflective of care needs.
• Develops mutually agreeable plans of care with patients, families, and healthcare teams.
• Facilitates safe transfers and ensures right care at the right time to improve quality and utilization.
• Maintains ongoing communication with interdisciplinary staff and services for transitions.
• Ensures regulatory and compliance standards are met in collaboration with the care team.
