Qualifications

• Bachelor’s degree in nursing or allied health, preferred.
• Minimum 2 years of progressive nursing experience in an acute care setting, preferably within discharge, utilization, and/or case management - or the equivalent in education and/or experience - is required.

Responsibilities

• Assess patients for transitional care needs and discharge barriers.
• Coordinate discharge plans involving community resources and alternate levels of care.
• Document plan of care and communicate with patients and families.
• Facilitate adherence to the discharge plan and ensure regulatory compliance.
