Job summary

Providence Medford Medical Center is seeking a part-time Care Manager RN - Social Work (0.4 FTE) to cover day shifts on weekdays and weekends. The RN Case Manager develops and manages patient care outcomes for a mixed-age caseload, performing assessments, care planning, discharge planning and utilization review.

Minimum qualifications include Oregon RN license and 3 years acute care hospital experience. Preferred BSN and 1 year in home health, mental health, substance use, hospice or utilization

Qualifications

Oregon Registered Nurse license required at hire.
3 years acute care hospital experience required.
Bachelor's Degree in Nursing preferred.
1 year in home health, mental health, substance use, hospice, and/or utilization review preferred.
Progressive nursing leadership experience, including charge nurse role, preferred.

Responsibilities

Develop and manage patient care outcomes for a caseload.
Perform assessment, treatment or care for patients across all ages.
Conduct complex discharge planning and utilization review.
