Job summary

UNC Medical Center in Chapel Hill is seeking an experienced RN to coordinate safe, timely discharges for patients with complex nutrition needs, including enteral feeding and TPN. The role focuses on risk assessment, care planning, and seamless transitions, with no night shifts or on-call.

Work Monday through Friday, 40 hours per week, onsite at UNC Medical Center. Requires at least 2 years RN experience; GI or surgical nursing knowledge is preferred.

Qualifications

• Minimum of 2 years of RN experience.
• GI or surgical nursing experience preferred.
• Strong communication and collaboration with care teams.
• Ability to coordinate safe discharge plans and advocate for patients.

Responsibilities

• Patient assessments to evaluate func­tional status and needs.
• Develop and implement patient-centered care plans.
• Identify barriers to care and address them.
• Coordinate care transitions across settings and teams.
• Communicate with payers to facilitate approvals and reimbursement.
• Document care and billing activities per requirements.
• Participate in quality improvement initiatives.
