Qualifications

• Must be a licensed RN in the state of North Carolina.

• Five (5) years of professional nursing experience.

Responsibilities

• Assesses patient status and coordinates care with an interdisciplinary team.
• Coordinates discharge planning and post-hospitalization care referrals.
• Collects and analyzes data for performance improvement and benchmarking.
• Educates patients and staff; documents teaching in the medical record.
• Promotes ongoing professional development and evidence-based practice.
